Interim Care Management Director - Hattiesburg, Mississippi

Position Overview

  • Role: Interim Care Management Director
  • Employment Type: Interim Leader
  • Location: Hattiesburg, Mississippi
  • Contract Length: 12 weeks

Key Responsibilities

  • Oversee implementation, development, and management of Care Management and Resource Management programs.
  • Ensure compliance with regulatory and accreditation standards (CMS, Joint Commission, etc.).
  • Lead and direct Care Management staff, ensuring accountability and adherence to scope of practice.
  • Conduct chart reviews to evaluate severity of illness and intensity of services for admissions, continued stays, and discharge planning.
  • Refer cases not meeting criteria to Physician Advisor or Utilization Management Committee and ensure follow-up.
  • Identify avoidable days and implement communication systems to resolve them.
  • Coordinate discharge planning and social services functions in compliance with federal/state laws and Joint Commission standards.
  • Act as liaison to community agencies for post-hospital placement and support.
  • Ensure timely reporting of Child Protective Services (CPS) or Adult Protective Services (APS) cases and educate staff on mandatory reporting.
  • Participate in Patient Care Conferences and Length of Stay meetings.
  • Perform other duties as assigned and comply with all policies and standards.

Qualifications

  • Bachelor’s Degree in Nursing (required)
  • Master’s Degree in Nursing, Healthcare Administration, or related field (preferred)
  • 4-6 years clinical nursing experience (required)
  • 3-5 years experience in care management or utilization review (required)
  • 1-3 years leadership experience in healthcare setting (required)

Skills and Abilities

  • In-depth knowledge of care management principles, discharge planning, and regulatory compliance.
  • Strong leadership and team-building skills.
  • Excellent communication and interpersonal skills for collaboration with multidisciplinary teams and external partners.
  • Analytical and problem-solving skills.
  • Proficiency with electronic medical records (EMR) and care management software.
  • Commitment to patient-centered care and continuous quality improvement.

Licenses and Certifications

  • Registered Nurse (RN) with State Licensure and/or Compact State Licensure (required)
  • Basic Life Support (BCLS) certification within 7 days of hire or transfer (required)
