Travel Registered Nurse (RN) - Case Management

Location: Lone Tree, Colorado
Employment Type: Travel
Contract Length: 13 weeks

Unit Information

Unit: Case Management
Shift: Day shift, 8:00 AM – 4:30 PM (may include weekend coverage)
Patient Ratios: 18-25 patients per case manager
Staffing: 2-3 case managers on the floor
Typical Team: 1 charge nurse, 3 RNs, 2 CNAs, 1-3 sitters
Floor: 5th floor, Main Hospital
EMR Systems: Meditech, CarePort

Job Description

Perform patient rounding focusing on high-risk discharge planning needs
Collaborate with physicians and nurses to develop safe discharge plans
Coordinate with skilled nursing facilities, home health, hospice, and LTACH providers
Arrange medication placement and transportation for patients
Conduct patient assessments and participate in patient-family conferences

Required Qualifications

Valid RN license in Colorado
Minimum 2 years of case management experience, including experience within the last year
BLS certification
Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
Experience in discharge planning
Experience working in home health, long term acute care (LTACH), skilled nursing facilities (SNFs), and hospice settings
