Registered Nurse - Home Health (Travel)

Location: Waukesha, Wisconsin

Contract Details

• Length: 13 weeks
• Shift: 10-hour day shifts (8:00 AM - 8:00 PM)
• Hours: 40 hours per week guaranteed (except during holidays)
• Rotation Example: Monday, Tuesday, Wednesday, Friday; or Monday, Tuesday, Wednesday, Saturday; or Sunday, Tuesday, Wednesday, Friday
• Patient Visits: Approximately 6 visits per day
• Travel: 6-county service area, ~250 miles per week (~50 miles per day)
• On Call: None

Job Requirements

• Experience:

  • Current Home Health and Hospice experience required
  • Recent OASIS experience required
• Certifications:

  • BLS (American Heart Association)
  • Wisconsin RN License
  • Valid Driver’s License
  • OASIS Certification is a plus
• Charting: Epic experience required within the last 12 months
• Patient Population: Adults 18 years and older with chronic disease management including CHF, COPD, Diabetes, Cancer, CVA, wound care, IV administration
• Patient Ratio: 1:1

Additional Information

• Must have home health and home hospice experience, including OASIS assessments
• Responsibilities include admission, death visits, and symptom management in hospice
• Must be physically able to tolerate driving throughout the day, navigating stairs, and uneven home environments
• Must be able to work in uncontrolled home environments (pets, smoke, allergies)
• Orientation will be during day shifts
• Candidate should be resilient, manage time well, adaptable to change, and socially engaging
• Dress code: Any scrub colors or corporate casual
• Float: May be required to float to other units/facilities based on census but only within clinical skills and with approval
• COVID-19 vaccine required unless medically or religiously exempt
