Overview
- Location: Brooklyn, New York
- Schedule: Day shift with at least one evening per week and some Saturdays
- Hours: 40 hours per week
- Contract length: 13 weeks
Role Summary
Registered Nurse role focused on diabetes education, patient counseling, and technical support for diabetes management devices and related care.
Required Qualifications
- Bachelor of Science in Nursing
- Valid RN license in New York
- Diabetes Education
- Diabetic Educator experience
- Clinic experience within the last year
- 1 year of clinic experience
Preferred Experience
- Familiarity with outpatient care and diabetes
Key Responsibilities
- Educate patients on diabetes management, including medications, insulin administration, blood sugar management, sick day rules, and glucagon pen use
- Provide counseling on nutrition and exercise
- Teach patients how to use, troubleshoot, and maintain continuous glucose monitors and insulin pumps
- Support data upload from home monitoring devices for in-person and virtual visits
- Assist with medication access issues, including prior authorizations, appeals, and coverage-related substitutions
- Respond to diabetes-related medical advice inquiries
- Collaborate with the diabetes care team to identify diabetes distress and support referrals
