Job Opportunity: Physical Therapist (PT) - Travel Position

Location: Hyden, Kentucky
Contract Length: 12 weeks

Job Description

• Must have 2+ years of inpatient Physical Therapy experience in an acute care setting.
• Coverage includes inpatient acute care and skilled nursing facility (SNF).
• Must hold a valid Kentucky license (or compact license valid in KY).
• Basic Life Support (BLS) certification required.
• Orientation occurs bi-weekly.
• Start date on job posting is the required start date.
• Candidate must reside within 100 miles of the facility to be considered.
• Assignment confirmation must be submitted within 24 hours of accepting the offer.
• Only submit candidate to one job at a time.

Required Certifications & Licenses

• BLS (Basic Life Support)
• ACLS (Advanced Cardiovascular Life Support) (required for most units except Psych and some Med/Surg)
• Kentucky or compact state license for Physical Therapist
• Proof of immunizations or immunity for:

  • MMR (Measles, Mumps, Rubella)
  • Hepatitis B (vaccination series or declination accepted)
  • Influenza vaccine (seasonal, October-March)
  • Varicella (Chickenpox)
  • Tdap (Tetanus, Diphtheria, Pertussis)
• TB testing: QuantiFERON blood test within one year prior to start or positive PPD with negative chest X-ray and questionnaire if applicable
• Annual physical or health statement signed by MD, DO, PA, or NP confirming clearance to work without restrictions

Professional Requirements

• Submission packet must include:

  • Cover sheet form
  • Resume
  • Skills checklist (clinical only)
  • Driver’s license
  • License/certification verification
• Minimum of 2+ years of experience must be documented with facility and unit information.
• Two professional references required:

  • One must be a charge or higher
  • One must be a supervisor reference within the last year
• Background check (ARH 20/20 Background Package) within 30 days prior to start, including:

  • 7-year county criminal history
  • Nationwide criminal, SSN trace, sex offender, SAM, OIG checks
  • Education and employment verifications
• Drug screen within 30 days of start (excluding marijuana/THC)
• Verification on Kentucky Vulnerable Adult Maltreatment Registry within 30 days prior to start
• Annual updates required for background, drug screen, immunizations, physical, and other credentials

Skills & Competency Requirements

• Skills checklist specific to scope of practice within one year prior to start
• Competency exam specific to scope of practice within one year prior to start (actual documents required, no certificates)
• Core competencies including HIPAA, patient rights, safety orientation, bloodborne pathogens, fire safety, infection control, elder abuse and neglect, age-specific care, cultural diversity, advance directives (annual update required)

• Prior travel experience is not explicitly required for this position.
• Candidate must complete and upload assignment confirmation form within 24 hours of offer acceptance or risk rejection.
