Job Description

• Position in Labor and Delivery unit (6 East) at Ascension Saint Alexius Medical Center

Required Credentials and Certifications

Licenses & Professional Credentials (Required for Submission)

• Current nursing license with Nursys verification within 30 days prior to start
• License/Certificate primary source verification within 30 days prior to start
• Driver's License or State ID (must be current and match address on file)
• Resume/Profile (Ascension Profile)
• At least two professional references
• Criminal Background Check (including Social Security Trace, Employment Verification, OIG Search, National Sex Offender)
• Employment Verification for past 7 years
• Education Verification relevant to clinical work (within 90 days of start)

Certifications (Required for Onboarding)

• BLS (American Heart Association)
• ACLS (American Heart Association)
• NRP (Neonatal Resuscitation Program, AHA/American Academy of Pediatrics)

Testing and Exams (Required for Onboarding)

• Ascension Pharmacology Exam (minimum 80%, updated every 3 years)
• Ascension Specialty Exam (minimum 80% or national benchmark, updated every 3 years)
• Ascension Core Competencies (HIPAA, Joint Commission NPSG, OSHA topics; minimum 80%, updated yearly)
• Ascension Cerner Test-Out (Cerner test results and training checklist with signed attestation)
• Ascension Skills Checklist (clinical skills, updated annually)

Onboarding Requirements

• Ascension Onboarding Education Attestation (6 hours pre-hire training)
• Ascension Nursing Contract Worker Onboarding (completed onsite)
• Ascension Biometric Consent
• Ascension Nova StatStrip Pre-Hire Modules
• Ascension Badge Photo (forward-facing headshot, white or blue background, no filters)

Health and Immunization Requirements (Required for Onboarding)

• MMR: Documented proof of two vaccines or positive IGG titer (no declinations accepted)
• Varicella: Proof of two vaccines, serological immunity, or history verified by healthcare provider (no declinations accepted)
• Tdap: Valid within 10 years (no declinations accepted)
• Hepatitis B: Vaccination series completion or signed declination
• Influenza Vaccine: Optional, but if declined during flu season (Oct 1 - Mar 31), mask required at all times
• Fit Test: Required for all travelers; preferred before start, can be completed onsite (annual update required)
• 2 Step PPD or IGRA (Quantiferon/Tspot): Required with specific timing before start; TB questionnaire updated annually
• Positive PPD: Negative chest X-ray within 1 year and annual TB questionnaire required
• Health Statement: Statement of good health or completed physical exam prior to first assignment, updated annually
• Ascension 12 Panel Drug Screen: Within 30 days prior to start, updated annually

• Some optional documents/checklists may be requested during or after onboarding (e.g., unit-specific checklist, hiring manager evaluation)
• All required documents must be current and compliant with Ascension Health policies
• Background checks and verifications must be updated annually or if there is a break in service longer than 90 days
