Job Description

• New position to support increased cardiac surgery volume.
• Productivity metrics indicate a busy and high-demand environment.
• Support includes virtual nursing, impacting staffing needs.

Required Certifications & Licenses

• BLS (Basic Life Support) – American Heart Association only, required for clinical positions.
• ACLS (Advanced Cardiovascular Life Support) – American Heart Association only.
• NIHSS (National Institutes of Health Stroke Scale) certification required.
• Permanent RN License – Primary source verification required within 30 days of start; temporary licenses not accepted.

Required Health & Immunization Credentials

• MMR (Measles, Mumps, Rubella) – Positive titers within last 15 years required; booster protocol applies if titers are low.
• Varicella – Positive titers within last 15 years required; booster protocol applies if titers are low.
• Hepatitis B – Proof of full series, partial series with declination, or positive titers within 15 years accepted.
• Tdap (Tetanus, Diphtheria, Pertussis) – Proof of vaccination within past 10 years required; declinations not accepted.
• 2-Step PPD or IGRA (Quantiferon/Tspot) – Negative test within last 6 months required; if positive, clear chest x-ray and TB questionnaire needed.
• Fit Test – Completed within 1 year of start; specific approved mask types listed.
• Influenza Vaccine – Proof required during flu season (Oct-Mar) or approved medical/religious exemption.

Other Required Documentation & Verifications

• Physical/Health Statement – Pre-employment physical exam within 1 year stating ability to work without restrictions.
• Criminal Background Check – National and county searches within 6 months; includes SSN trace and NSO.
• Ohio Fingerprints – BCI&I check and FBI fingerprint check required depending on residency duration in Ohio.
• Education Verification – Highest level of education verified; international education requires equivalency documentation.
• Employment Verification – Verification of employment within last 5 years through third-party.
• Social Security Trace and National Database Criminal Background Check – As applicable.
• Drug Screen – 10-panel drug screen within 60 days of start; instant drug screens not accepted.
• UHS Competency Exam – Must score at least 80%, completed within 1 year prior to start.
• UHS Core Competencies – Includes HIPAA, Joint Commission, OSHA topics; 80% passing score required.
• UHS Skills Checklist – Clinical skill-specific checklist completed within 1 year prior to start (initial hire only).
• UHS I-9 Attestation – Proof of eligibility to work in the U.S.
• UHS Submission Packet – Resume including education and work history, 2 professional references within last 2 years, and completed job application.
• UHS Badge Photo – Professional headshot (shoulders up, plain background, JPEG format).
• UHS Module Completion Certificate – Completion of safety and compliance training modules prior to clearance.

Optional / Preferred

• COVID-19 Vaccine – Preferred but not required; medical or religious exemptions accepted with proper documentation.
• Annual Negative PPD – If 2-step PPD not done within past year.
• Positive PPD with Negative Chest X-Ray and Annual TB Questionnaire – If applicable.
