Schedule

• Shift: Nights, 6:30 PM to 7:00 AM
• Hours: 12-hour shifts, 3 shifts (more if desired)
• Likely Days: Thursday, Friday, Saturday
• On-call: No call

• Polysomnographer with 1 year of experience, completed within the last year
• BRPT
• BLS

Onboarding Requirements

Health and Immunizations

• COVID-19 vaccine: Proof of September 2025 booster, or a declination or exemption on agency letterhead. Declinations are not accepted in CA or IL. End date of October 2026.
• MMR: Proof of 2 vaccinations with dates, or a primary source IgG titer with lab printout.
• Varicella: Proof of 1 vaccination with date, or a primary source IgG titer with lab printout.
• Hepatitis B: 3 doses, or a declination.
• Tdap: Vaccine within the past 10 years. Declinations are accepted only where the site allows and the assignment does not involve pediatric interaction. Agency form acceptable for declination where allowed.
• Influenza vaccine: Proof, declination, or exemption on agency letterhead. Declinations are not accepted in CA or IL. Flu seasons: 10/1/2024-4/30/2025 and 10/1/2025-4/30/2026.
• TB screening: Negative TSPOT or QuantiFERON within a year of start, or a 2-step PPD with both reads within the last year. Positive results with a chest X-ray within a year of start are accepted.
• Fit test: 3M 1860 or 1870.
• Physical/health statement: Exam documentation from within the last year showing fitness to work.
• 10-panel drug screen: Completed within 30 days of start (preferably within 14 days), resulted with MRO signature before start, and must include the listed panel. THC is not included.

Professional and Administrative

• Profile: Resume, skills checklist within a year, and 2 professional references (submission required).
• Confidentiality and system security agreement: Page 5 signed and on file.
• Conflict of interest disclosure: Page 1 signed and on file.
• Criminal background check: Federal, nationwide, and county checks covering the last 7 years, plus SSN verification. Dated within 30 days of start. Check all addresses and AKA names.
• Education verification: Third-party report of highest education level. If a specific certification is required, it must be provided at onboarding.
• EPLS and OIG/GSA exclusion checks: Dated within 30 days of start. All aliases searched.
• NSO (National Sex Offender Registry): Dated within 30 days of start. Use nsopw.gov. Michigan also requires the statewide check.
• Iowa registry check: Criminal history, sex offender, dependent adult, and child abuse registries. Must be completed within 30 days before start.
• OFAC: Completed within 30 days of start.
• Integrity and audit services form: Signed within days before start.
• Non-employee upload form: Iowa, Des Moines/West Des Moines. Complete fully and upload pages 1-3 and 28.
• Code of conduct form: Page 27 signed and on file.
• Child and adult dependent abuse training: Required in Iowa.
• I-9: Completed by the agency.
• System access form: Completed and uploaded one week before start.

Testing and Competency

• Joint Commission CORE exam: Within a year.
• Specialty exam: Copy required, completed within a year.
• Skills checklist: Provided by the agency for clinical contractors.
