Physical Therapist - Home Health (Travel/Contract)

Job Details

• Shift: Monday to Friday, 5x8 hour days (8:00 AM - 4:30 PM, start time flexible)
• Schedule: Day position, no weekends or on-call required
• Hours: Guaranteed 40 hours per week
• Setting: Community-based care in patients' homes with office on campus
• Average Daily Census: 26 patients
• Floating: May float to hospice within scope if needed; no floating to sister facilities required
• Block Schedule: Accommodated (Monday-Friday)
• Locals Accepted: No

Required Experience & Skills

• Years of Experience: Minimum 1 year in Home Health or Orthopedic/Aging (OA)
• Epic Experience: 1 year preferred
• Prior Travel Experience: Not explicitly required
• First-Timers: Accepted if OASIS experience is present
• Certifications:

  • Alaska Physical Therapist License (Compact PT allowed)
  • Basic Life Support (BLS) required
• Pending State License: Alaska PT License required before start
• Vehicle: Reliable vehicle needed

Education

• Bachelor's, Master's, or Doctorate Degree in Physical Therapy from an accredited program (or foreign equivalent)

Job Responsibilities

• Evaluate patients and establish treatment plans and goals
• Provide patient treatment and modify plans as needed
• Complete documentation accurately and timely per regulatory and licensure standards
• Collaborate with healthcare team members to ensure continuous patient care
• May supervise therapy assistants, aides, volunteers, and students
• Participate in quality improvement, staff meetings, and educational activities
• May serve as an educational resource and mentor to new caregivers

• COPD
• Congestive Heart Failure (CHF)
• Orthopedic procedures including joint replacements
• Neurological diseases

Licensure & Compliance Requirements

• Must sign Social Security verification release
• Background checks required within 60 days of start (7-year search including SSN trace, counties/states lived/worked, federal and national criminal checks)
• EPLS/GSA/SAM and HHS/OIG checks required before start and every 45 days thereafter
• Social Security verification required before or pending start
• State-specific background checks as applicable (e.g., Washington State Patrol for WA placements)
• Physical exam within 1 year of start
• Fit mask test within 1 year and updated annually (if onsite)
• Immunizations and/or declinations required for:

  • Tetanus/Diphtheria/Tdap
  • MMR (Measles, Mumps, Rubella)
  • Varicella
  • Hepatitis B
• Drug testing required within 30 days of start (specific panels required)
• Seasonal flu vaccination or approved declination required during flu season
• Respiratory safety clearance required for onsite candidates

Orientation & Training

• Must complete PeaceHealth-specific orientation modules and pass related exams with 100% score prior to start
• Orientation includes review of policies such as dress code, masking, personnel restrictions due to illness, and daily responsibilities
• Must complete and submit various facility-specific forms and checklists prior to start
• ID badge photo and parking forms required before start

• No on-call or weekend work required
• May provide oversight and clinical instruction to therapy assistants and students
• Must be flexible to meet facility needs within scope of practice
• Ketchikan, AK offers mild winter climate moderated by the Pacific Ocean, similar to Seattle, WA
