- Guaranteed Hours: 36 hours per week (client may cancel up to 3 shifts during assignment)

Required Background and Documentation
- Valid Driver’s License/State ID (front and back copies)
- Background Check covering last 7 years, including:
- Social Security Address Check (last 15 years)
- National Criminal Search
- Sex Offender Search
- Education Verification
- International Searches (if applicable)
- EPLS/GSA/SAM check within 30 days prior to start
- HHS/OIG check within 30 days prior to start
- Education verification
- State-specific Medi-Cal Exclusion Search (California)
- Professional License primary source verification within 30 days prior to start (active status, issue and expiration dates)
- NURSYS verification for RNs
Health and Immunization Requirements
- Physical exam within 12 months clearing employee to work without restrictions (updated annually)
- Fit Mask test (N95 or approved alternatives) within required timeframe
- Tdap vaccine or declination within 10 years
- Negative TB test or chest X-ray per facility protocol
- TB Questionnaire (required for all candidates)
- Immunity or vaccination records for:
- Rubeola (Measles)
- Mumps
- Rubella
- Hepatitis B
- Varicella
- Hepatitis C
- Negative 10-panel drug screen within 30 days of start (no instant tests)
- Annual Medical Release Form
