Required Certifications

• ACLS (Advanced Cardiac Life Support) - AHA only

• TNCC (Trauma Nursing Core Course)
• PALS (Pediatric Advanced Life Support) - for some units
• NRP (Neonatal Resuscitation Program) - for some units
• EFM (Electronic Fetal Monitoring) - for L&D units

Required Background and Documentation

• Valid Driver’s License/State ID (front and back copies)
• Background Check covering last 7 years, including:

  • Social Security Address Check (last 15 years)
  • National Criminal Search
  • Sex Offender Search
  • Education Verification
  • International Searches (if applicable)
• EPLS/GSA/SAM check within 30 days prior to start
• HHS/OIG check within 30 days prior to start
• Education verification
• State-specific Medi-Cal Exclusion Search (California)
• Professional License primary source verification within 30 days prior to start (active status, issue and expiration dates)
• NURSYS verification for RNs

Health and Immunization Requirements

• Physical exam within 12 months clearing employee to work without restrictions (updated annually)
• Fit Mask test (N95 or approved alternatives) within required timeframe
• Tdap vaccine or declination within 10 years
• Negative TB test or chest X-ray per facility protocol
• TB Questionnaire (required for all candidates)
• Immunity or vaccination records for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B
  • Varicella
  • Hepatitis C
• Negative 10-panel drug screen within 30 days of start (no instant tests)
• Annual Medical Release Form
