• CPI (Crisis Prevention Institute) for ER, Surgical & Trauma ICU, Trauma Stepdown, Behavioral Health (CPI Blue Card required; online courses not accepted)
• NIH Stroke Certification (as applicable per job description)
• Pass/Fail Testing (EKG, Medication Calculation, Blood Administration) required for nursing candidates
• State License Verification (Primary Source Verification within 30 days of start)
• NURSYS Verification within 30 days of start
• Other certifications as required per unit job description

Background Checks & Compliance

• 7-year criminal background check within 30 days of start (includes SSN trace, county, sex offender, national, and international searches if applicable)
• EPLS/GSA/SAM, HHS/OIG, and National Sex Offender Registry checks within 30 days of start and updated annually (must be completed independently)
• Drug screen within 30 days of start and updated annually (includes multiple substances; no placeholders or pending results allowed)

Health & Immunization Requirements

• Physical exam within 1 year of start, signed by appropriate medical professional, confirming good health and no limitations
• Annual fit mask testing (3M N95 1860 Small or Regular and Aura 9205 only); onsite fit testing may be accommodated if no clinic within 100 miles
• Tetanus/Diphtheria/Tdap vaccination (given after age 11) required; medical and religious exemptions accepted with approval
• Negative TB test within 3 months of start (2-step skin test or preferred QGold/TSpot); annual TB questionnaire for positive responders
• Immunizations or positive titers required for:

  • Rubeola (Measles)
  • Mumps
  • Rubella
  • Hepatitis B (series or declination with documentation)
  • Varicella
• Seasonal Flu vaccination or approved exemption required annually (strict deadlines apply)
• COVID-19 vaccination not mandatory; proof of vaccination or signed declination accepted

Documentation & Orientation

• Valid Driver’s License required
• Social Security Card required (legible)
• Residency Declaration Form if working on a compact license (driver’s license must match compact state)
• Various facility-specific documents to review, initial, and return (dress code, blood administration policy, math calculation packet, EKG review, time and attendance, medical release, vaccination exemption forms, application request, delegate form, state liability and time gap forms)
• Orientation is two days (Thursday and Friday) including Cerner Infusion Management
• Inservice training modules (including pass/fail testing) must be completed approximately 1 week before start (estimated 4 hours)
• Annual skills checklist required for specialty; MS/Tele skills checklist required for Clinical Decisions unit
• One professional reference required from a manager, supervisor, or charge nurse within 1 year of start

Additional Notes

• Candidates cannot start with pending certifications or incomplete required documents.
• Candidates with gaps in service of 30 days or more may require new background checks and completion of certain documents again.
• Latex allergy must be disclosed to avoid exposure.
• PAPR accommodations available if unable to pass fit testing due to facial hair or face shape.
