Job Opportunity: LVN / LPN - Travel/Contract

Location: Utica, New York
Contract Length: 13 weeks


Job Description

  • Position: Licensed Practical Nurse (LPN)
  • Required Experience: Minimum 1.5 years in LPN/LVN role
  • Required License: New York State LPN license
  • Experience in Rehabilitation Nursing Center and Long-Term Care (LTC) settings
  • Schedule: Every other weekend
  • EMR System: PointClickCare preferred
  • Local radius: 75 miles (local candidates accepted)

Required Certifications

  • ACLS (Advanced Cardiac Life Support) – AHA strongly preferred
  • BLS (Basic Life Support) – AHA strongly preferred
  • PALS (Pediatric Advanced Life Support) – AHA strongly preferred
  • TNCC (Trauma Nursing Core Course) – Required for ER staff (if applicable)
  • State License verification (primary source online verification)
  • NIH Stroke Certification (if working in ICU, PCU, ER, CT ICU, or specific units)

Required Competencies

  • Medication Test/Pharmacology Competency Exam (minimum 80% score or meet Angoff score)
  • Competency Exam (minimum 80% score or meet Angoff score)
  • Dysrhythmia/EKG Competency Exam (required for ICU, PCU, ER, CT ICU, and certain units; expires every 3 years)

Health and Background Requirements

  • Physical Exam within 1 year of start
  • Fit Mask test within 1 year of start, updated annually (specific N95 masks required)
  • Negative TB test or Chest X-Ray within 1 year of start (PPD, Quantiferon Gold accepted; Tspot NOT accepted)
  • Immunizations or positive titers required for:
    • Rubeola (Measles)
    • Mumps
    • Rubella
    • Hepatitis B (titer, declination, or vaccine series)
    • Varicella (positive titer, vaccine, or documented chickenpox history)
  • Drug Testing: 9 Panel Drug Screen within 1 year of start
  • Seasonal Flu Vaccination or Declination (required during flu season: 9/1 - 3/31)
  • Pneumonia Vaccination or Declination (required for all candidates, especially rehab travelers)

Documentation and Orientation

  • Government issued photo ID (Driver’s License or State ID)
  • Emergency Contact Form (required prior to start)
  • Background Checks:
    • Standard 7-year search including SS Trace, County searches, Nationwide, Sex Offender Registry
    • EPLS/GSA/SAM and HHS/OIG checks (annual)
    • Education Verification and Florida Education Audit Confirmation (for RN/LPN staff)
  • Facility-specific documents to review and sign:
    • Job Description
    • Code of Conduct
    • Sexual Harassment Prevention Training
    • Code Elopement Handout
    • New Employee Orientation Booklet
    • Welcome Packet acknowledgment
    • CHRC Form (for allied travelers)

  • Online learning modules must be completed prior to first shift
  • Photo for badge: appropriate headshot required
