Job Description

• Outpatient facility setting
• Pediatrics experience required
• Schedule: 4x10 day shifts
• No weekend shifts
• Required certifications/licenses:

  • BLS (Basic Life Support)
  • CCC-SLP (Certificate of Clinical Competence in Speech-Language Pathology)
  • Florida State License
• Minimum 2 years of experience required in specialty

Required Experience

• Specialty: Minimum 2 years of experience in Speech-Language Pathology
• Prior travel experience: Not explicitly required

Certifications & Licenses

• BLS (AHA or ARC, Provider or Instructor, RQI accepted)
• CCC-SLP certification
• Florida State License (Primary Source Verification required)

Compliance & Background Requirements

• Background check within 30 days of start, covering all locations lived/worked/schooled in past 7 years
• EPLS/GSA/SAM and HHS/OIG checks required within 30 days of start
• Education verification (Diploma or Transcript acceptable)
• Fingerprinting: Florida AHCA Level II fingerprint clearance required prior to start (Livescan electronic fingerprinting)
• Drug testing: 10-panel drug screen within 30 days of start (Amphetamines, Cocaine, Opiates, PCP, THC, Benzodiazepines, Barbiturates, Methadone, Propoxyphene, Methaqualone)
• Physical exam within 6 months of start
• Fit mask test (N95) within 1 year of start, updated annually
• Immunizations and titers required:

  • Tdap within 10 years
  • Negative TB test (Qgold/TSPOT within 1 year; PPD not accepted)
  • Measles, Mumps, Rubella (2 MMR vaccines or positive titer)
  • Hepatitis B (series, titer, or signed declination)
  • Varicella (2 vaccines, titer, or history of disease)
• Seasonal flu vaccination or approved medical/religious exemption required annually

Additional Requirements

• 2 professional references
• Employment application and work history verification (7 years or last 3 employments relevant to profession)
• Skills checklist within 1 year of start
• Facility-specific documents including:

  • Affidavit of Good Moral Character (notarized)
  • Confidentiality Agreement
  • Handbook Attestation Form and Quiz
  • Contractor Attestation Form
  • Flu exemption forms if applicable

Schedule & Work Environment

• 4 days per week, 10-hour day shifts
• No weekend work
• Outpatient pediatric therapy setting
