Required Certifications

• BLS (Basic Life Support) – Required for all candidates; must be AHA provider or AHA RQI (RQI from other providers not accepted)
• CPI (Crisis Prevention Institute) – Required for Behavioral Health clinical candidates; may be completed onsite twice per month
• Additional certifications such as ACLS, PALS, NRP, TNCC, EFM may be required or recommended depending on unit but are not mandatory for this role

Required Licensure

• Must hold a valid Registered Nurse (RN) license for Illinois
• Primary source license verification required within 30 days of assignment start
• NURSYS verification required within 30 days of assignment start

Required Documents & Checks

• Valid Driver’s License or State ID
• Background checks including 7-year history, aliases, sex offender, national and federal criminal checks, education verification, and others as applicable
• Education verification: ADN or BSN required; diploma/transcripts acceptable if verification unavailable
• Physical exam within 1 year, updated annually
• Fit mask test within 1 year (specific mask models required)
• Immunizations and titers (must be current or have valid declinations):

  • Tetanus/Diphtheria/Tdap (within 10 years)
  • Negative TB test or chest X-ray (within 1 year)
  • Measles, Mumps, Rubella (MMR) proof or titers
  • Hepatitis B vaccination or immunity proof (declination allowed)
  • Varicella vaccination or immunity proof (declination allowed)
• Drug testing: Negative 9-panel within 30 days of start
• Flu seasonal vaccination/documentation or approved medical/religious exemption (seasonal requirement)
• Fingerprinting and Illinois Department of Public Health registry enrollment if no Illinois state license required (not applicable for RN)

Additional Requirements

• Completion of facility-specific orientation modules prior to start (approximately 2 hours)
• Photo for badge (passport style, plain background, no headgear)
• Employment application including 7 years of work history with explanations for gaps over 30 days
• Two professional references required
• Skills checklist updated annually for assigned and float units
• Work history verification covering 7 years of employment

Schedule & Work Environment

• Floating to other units required
• Every other weekend work required
• No block scheduling available
• Use of Epic electronic health record system

Notes

• Prior travel experience is not required for this position.
• Pending licenses are not accepted.
• Religious and medical exemption forms for COVID-19 and influenza vaccines must be completed and approved at least one week prior to documentation due date if applicable.
