Job Opportunity: Home Health Nurse (LVN / LPN)

Location: Omaha, Nebraska
Employment Type: Travel/Contract
Contract Length: 13 weeks


Job Description

  • Provide specialized wound and ostomy care in a clinic setting focused on chronic, non-healing, or slow-healing wounds.
  • Patient population includes those with diabetic wounds, pressure wounds, vascular wounds, non-healing surgical wounds, and ostomy care needs.
  • Services include wound care, patient education, pre-surgical marking, post-surgical follow-up, counseling, and coordination of advanced therapies such as negative pressure wound therapy, compression therapy, biological dressings, hyperbaric oxygen therapy, and advanced dressings.
  • Work with a multidisciplinary team of specialists.

Work Schedule

  • Monday to Friday, 8:00 AM to 4:30 PM
  • No weekends, holidays, or on-call required
  • 40 hours per week

Required Qualifications

  • Valid Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) license in Nebraska
  • Minimum of 1 year of home health nursing experience within the last year

Compliance & Documentation Requirements

  • Government-issued photo ID required
  • Background check within 30 days of start, including multi-county and federal checks
  • Ongoing monthly SAM and OIG checks required
  • Education verification required
  • State-specific registry checks required (Nebraska DHHS Adult & Child Services Registry)
  • Primary source license verification and NURSYS verification required
  • Annual physical exam and fit mask test (3M 1860 Small/Regular or 3M 1870+) required
  • Immunizations and health screenings required:
    • Tdap within last 10 years
    • Annual TB testing or chest x-ray if history positive
    • MMR (2 vaccines or positive titer; boosters if low/equivocal)
    • Hepatitis B titer or declination
    • Varicella (2 vaccines or positive titer; boosters if low/equivocal)
    • Drug screening within 60 days of start and annually (specific panel required)
    • Flu vaccine preferred (documentation or declination required)
    • COVID vaccine preferred (medical/religious exemption or declination accepted)
  • Annual skills checklist required
  • Two professional references from the past 2 years required

Additional Notes

  • Orientation Safety Manual acknowledgment required
  • Eye exam required only if working in the OR (not applicable here)
  • Locals not accepted; candidate must be at least 100 miles away from the facility
  • Return to office (RTO) requests must be submitted with application; additional requests subject to manager discretion
