Physical Therapist - Home Health (Travel/Contract)

Location: Pomona, California (territory includes Corona, Riverside, Moreno Valley, Norco, Eastvale, Jurupa Valley, Perris, Redlands)
Contract Length: 13 weeks
Shift: Day shift, 8:00 AM – 5:00 PM (5 days/week, 8 hours/day)
Guaranteed Hours: 40 hours/week

Job Overview

• Provide physical therapy services in a home health setting within a 60-mile radius of the office hub.
• Average caseload: 5-7 patients per day, with a minimum of 6 visits daily or 30 therapy points weekly.
• Patient population primarily geriatric.
• Timely communication and documentation are required.
• Dress code: Scrubs (any color), no jeans.
• iPad provided for documentation; cell phone use not allowed during visits.
• Mileage reimbursed at IRS standard rate.
• On-call duties: Not required.
• Local candidates accepted at travel rates.

Required Qualifications

• Valid Physical Therapist license in California.
• Minimum 1 year of recent physical therapy experience within the last year.
• Experience with OASIS.
• Experience with Homecare Homebase (HCHB) electronic medical record system.
• Basic Life Support (BLS) certification (AHA, ARC, or Military accepted).

Compliance and Background Requirements

• Background checks must be current within 90 days of start and updated annually, including:

  • SSN Trace
  • County searches for last 7 years of residence/work/school
  • FACIS Level 1
  • Nationwide Sex Offender Registry
  • Motor Vehicle Records/Driver’s Record
  • Education verification (highest degree related to profession)
  • Work history verification (7 years, with 3 attempts if unverifiable)
• EPLS/GSA/SAM and HHS/OIG checks required within 90 days of start.
• Fingerprinting requirements vary by state; candidates must comply as applicable.
• State-specific criminal history and employability status checks required within 90 days of start.

Health and Safety Requirements

• Physical exam within the last 12 months (not required annually).
• Fit test for N95 mask required (no specific model preference).
• Negative TB test (PPD, Q Gold, or TSpot) within 1 year or chest X-ray within 5 years if positive PPD. Annual TB questionnaire required.
• Immunizations or titers required for:

  • Measles (Rubeola)
  • Mumps
  • Rubella
  • Hepatitis B
  • Varicella
• Seasonal flu vaccination or facility-specific declination form required during flu season.
• COVID vaccination or facility-specific declination required.
• 9-panel drug screen within 12 months of start (instant acceptable).

Documentation and Other Requirements

• Must provide proof of current auto insurance (required for all home health positions involving driving).
• Completion of facility-specific Acceptable Use Policy prior to start.
• Two professional references within 2 years of start (travel, allied, per diem acute care).
• Full 7 years of work history documented in application with no gaps.
• Annual skills checklist and competency exams as applicable.
