Required Qualifications

Valid Registered Nurse (RN) license in Pennsylvania
Associate of Science in Nursing (ASN) / Associate Degree in Nursing (ADN)
Minimum 1 year of Behavioral Health experience, with experience within the last year required
Experience in an inpatient setting
Basic Life Support (BLS) certification issued by the American Heart Association
Advanced Cardiac Life Support (ACLS) certification
Pediatric Advanced Life Support (PALS) certification issued by the American Heart Association
NIH Stroke Scale (NIHSS) certification
Crisis Prevention Institute (CPI) certification

Experience

Required Documentation & Compliance

- Pennsylvania Statewide background check (Patch Certificate)
- EPLS/GSA/SAM and HHS/OIG checks
Pennsylvania Department of Human Services fingerprinting results (required prior to start)
Physical exam within 1 year of start
Negative TB test (PPD, QuantiFERON Gold, or T-spot) within 1 year or negative chest X-ray if previously positive
Annual TB questionnaire for past positive TB status
Immunizations or titers within 10 years for:
- Rubeola (Measles)
- Mumps
- Rubella
- Varicella
- Hepatitis B (vaccine series, positive titer, or signed declination)
Negative 15-panel drug screen within 30 days of start
Seasonal flu vaccination or accepted declination during flu season (Oct 1 – Apr 1)
Facility-specific documents and attestations to be completed prior to start, including:
- Orientation checklist
- Pandemic attestation
- Code of conduct
- Confidentiality agreement
- Disclosure statement
- Assessment of patient policy, interdisciplinary plans of care, skin integrity, mobility, catheter training, and cardiac procedure education (all require proof of completion and passing quizzes with 100%)
Photo for badge required

Additional Notes

Workplace Violence Prevention (CPI) training will be completed onsite during orientation; certificate not provided beforehand
Facility will provide first week’s schedule and direct contact within 48 hours of start date
Two professional references required for travel or acute care roles (within 3 years)
Skills checklist required annually
