Job Opportunity: PT Home Health (Hospice Therapy/Rehabilitation)

Job Details

• Specialty: Hospice Physical Therapy
• Patient Load: 5-7 patients per day (geriatric cases)
• Shift: Day shift, 8:00 AM to 5:00 PM (5 days a week, 8 hours per day)
• Guaranteed Hours: 40 hours per week
• Territory: Up to a 60-mile radius from the office hub
• Dress Code: Scrubs (any color), no jeans
• Technology Provided: iPad (cell phone use not allowed)
• Mileage Reimbursement: Yes, at IRS standard rate
• On-call Requirement: Yes, every 6th weekend
• Flexibility: Traveler expected to be flexible with territory
• Timely documentation and communication are required
• First-time travelers are not accepted
• Local candidates are not accepted

Required Qualifications

• Valid Physical Therapist license in Ohio
• Minimum 1 year of Physical Therapist experience within the last year
• Completion of at least one travel assignment
• Basic Life Support (BLS) certification
• Experience with Homecare Homebase (HCHB) Electronic Medical Record system

Background and Compliance Requirements

• Background checks must be completed within 90 days of start and updated annually, including:

  • SSN Trace
  • County searches for last 7 years of residence/work/school
  • FACIS Level 1M
  • Nationwide Sex Offender Registry
  • Motor Vehicle Records (MVR)
  • Education and Work History Verification
  • State-specific searches as applicable (e.g., Kentucky, Virginia, Indiana, New Hampshire)
• Ohio-specific fingerprinting (BCI and FBI) required within 12 months of start and updated annually; reprints required if candidate has lived/worked outside Ohio since last fingerprinting
• EPLS/GSA/SAM and HHS/OIG checks required within 90 days of start
• Education verification of highest degree related to profession required
• Signed Orientation Safety Manual Acknowledgement (updated annually)
• Medication/Pharmacology competency exam as applicable
• Agency background attestation required prior to start

Health and Immunization Requirements

• Physical exam within the last 12 months (not required annually)
• Negative TB test (PPD, Q Gold, or TSpot) within 1 year of start; chest X-ray within 5 years if positive PPD
• Annual TB questionnaire required
• Immunizations or positive titers required for:

  • Measles (Rubeola)
  • Mumps
  • Rubella
  • Hepatitis B
  • Varicella
• 9-panel drug screen within 12 months of start (instant acceptable)
• Seasonal flu vaccination or facility-specific declination form required during flu season
• COVID vaccination or facility-specific declination required

Documentation and History

• Full 7 years of work history with no gaps, including city and state of employment
• Employment history verification for 7 years, with 3 attempts required if unverifiable before attestation accepted
• Two professional references required within 2 years of start (for travel, allied, per diem acute care roles)
• Skills checklist required annually
• Proof of current auto insurance required (due to driving responsibilities in home health)
