Job Description

• Provide home health physical therapy services in two main areas covering multiple cities including Fremont, Hayward, Oakland, Berkeley, and others.
• Shift: 8-hour day shifts (8:00 AM - 4:30 PM), 5 days per week.
• Expected to see 5-6 patients per day with a standard productivity target of 27 points.
• Guaranteed 36 hours weekly (note: guaranteed hours do not apply during the first 2 weeks, holiday weeks, or time off weeks).
• Weekend patient visits are optional to meet productivity goals.
• No on-call responsibilities.
• Own transportation is required; occasional travel radius may exceed 50 miles.
• Timely communication and documentation are mandatory; documentation tools will be provided and must be returned.
• Scrub color is flexible (any color allowed).
• Free parking available.
• Mileage reimbursement follows IRS standard rates.

Required Qualifications

• Valid Physical Therapist license in California.
• Minimum 1 year of Physical Therapist experience, including experience within the last year.
• Experience in a Home Health setting.
• Completion of at least one travel assignment as a Physical Therapist.
• Current Basic Life Support (BLS) certification.

Compliance and Onboarding Requirements

• Government-issued photo ID (Driver’s License or State ID).
• Emergency contact information form.
• I-9 attestation and standard background checks including 7-year work/location history, sex offender registry, and federal exclusions (updated annually).
• Annual EPLS/GSA/SAM and HHS/OIG checks.
• Additional certifications such as ACLS and PALS are noted but not explicitly required for this role.
• Verification of professional license through primary source.
• Auto insurance and current vehicle registration required and must be maintained throughout the assignment.
• Physical exam within 1 year prior to start, confirming clearance to work without restrictions.
• Immunizations and health screenings required:

  • Tetanus/Diphtheria/Tdap (within 10 years or annual declination)
  • Negative TB test or chest X-ray within 1 year
  • Measles, Mumps, Rubella (2 vaccines or positive titers within specified timeframes)
  • Hepatitis B (titer, vaccine series, or declination)
  • Varicella (2 vaccines or positive titer)
  • Seasonal flu vaccine or declination (required year-round)
  • COVID-19 vaccination documentation or facility-specific declination form (religious/medical exemptions accepted)
• 10-panel drug screen within 1 year of start.
• Work history verification of at least 1 year related to profession.
• Two professional references within 3 years for travel/allied/per diem acute care roles.
• Annual skills checklist and competency exams as applicable.

Additional Notes

• The system attempts to group patient visits to minimize driving time.
• Training provided for documentation systems used (MediScrib & OASIS).
• Locals are accepted for this travel assignment.

This position is ideal for experienced home health physical therapists licensed in California who have prior home health experience and are comfortable managing a full patient caseload with travel between multiple locations.
