Job Description

The Registered Nurse (RN) will coordinate and direct the provision of home nursing care according to physician's orders, agency policies, and procedures. Responsibilities include:

• Application of the nursing process in home health settings.
• Direct supervision and evaluation of Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.

Travel Requirements

• Home visits and periodic office meetings.
• Must have reliable transportation, a current driver's license, and agency-required liability insurance.
• Willingness to travel up to 75 miles from the home clinic.

Patient Care

• Average number of patients per day: 5-8.
• Points system for productivity: 30 points per week, with each type of visit and activity holding a different point value.

Types of Cases

• Start of Care
• Wound Care
• Follow-up Visits

Qualifications

• Experience Required: Minimum of 1 full year of Adult Home Health or Hospice experience.
• Certifications Required: CPR, BLS.
• On-call Requirements: Varies by location and profession; to be discussed during the interview.
• First-timers Accepted: No, must have recent specialty experience.

Schedule

• Standard hours: Monday to Friday, 8 AM to 5 PM.
• Weekend requirements vary by location and profession.

Charting and Documentation

• Proficiency in HomeCare HomeBase (HCHB) and Oasis is required.
• Must have used HCHB within the last 6 months or be willing to take a retraining course.

Additional Information

• Dress Code: Scrubs.

• Pending License: Not accepted; must have license in hand.
