Overview

Physical Therapist (PT) role providing evaluation, rehabilitation, treatment planning, documentation, and discharge planning for patients with physical functioning disorders and/or wound care.

Required Qualifications

• Valid Physical Therapist license in Missouri
• 1 year of Physical Therapist experience
• Physical Therapist experience within the last year
• Experience in a Long Term Care or Rehabilitation setting

Preferred / Additional Notes

• New grads are okay if they have Level II experience; Level I observation experience alone is not sufficient

Responsibilities

• Screen, evaluate, and rehabilitate patients
• Develop and adjust plans of care
• Use standardized assessments and evidence-based practice
• Provide manual techniques, ambulation, therapeutic exercises, modalities, supportive and assistive devices, and wound care
• Document treatment and required clinical information in the electronic documentation system
• Record treatment minutes
• Participate in required meetings, trainings, in-services, and conferences
• Obtain necessary DME or supplies
• Supervise and support physical therapist assistants, rehabilitation aides, and/or students
• Communicate with supervisor, team members, patients, families, caregivers, and other service providers
• Provide consultation, counseling, and discharge summaries
