Registered Nurse (RN) - Long Term Care (LTC)

Location: Ludington, Michigan
Contract Length: 13 weeks

Facility & Unit Information

• Type of Facility: Skilled Nursing Facility (SNF)
• Beds/Residents: 83 beds / 74 residents
• Dementia Unit: 20 beds in a locked down dementia unit
• EMR System: Point Click Care
• Nurse-to-Patient Ratios:

  • Days & Afternoons: 1 RN per 15 residents (5 RNs for 74 residents)
  • Midnights: 1 RN per 74 residents

Required Certifications & Experience

• Required Certifications: BLS (AHA), RN License
• Specialty Experience: Minimum 1 year Long Term Care (LTC) experience required
• Prior Travel Experience: Not required; open to first-time travelers

Patient Population & Common Diagnoses

• Common Diagnoses: Congestive Heart Failure (CHF), Chronic Obstructive Pulmonary Disease (COPD), Diabetes Mellitus (DM), Rehabilitation, Dementia
• IV Therapy: IVs accepted for fluids and antibiotics
• Respiratory Therapy: No on-site RT 24/7; Bipap and Cpap patients only, no vented patients

Job Duties & Responsibilities

• Administer medications
• Perform head-to-toe assessments
• Possible charge nurse duties
• Floating throughout the facility

Scheduling & Work Hours

• Shift Options:

  • 8-hour shifts:

    • Days: 7am - 3pm
    • Midnights: 3pm - 11pm
    • Nights: 11pm - 7am
  • 12-hour shifts:

    • Days: 7am - 7pm
    • Nights: 7pm - 7am
• Block scheduling allowed for agency nurses
• Weekend Requirement: Every other weekend
• Holiday Requirement: May be asked to work holidays
• Call/Standby: None

Orientation & Training

• Orientation includes Relias modules, skills checkoff, and two days on the general side of the unit

Additional Notes

• No telemonitors on unit
• No pharmacy or medication dispensing system on-site
• RN attendance at team conferences for acute inpatient rehab not required
• 24-hour physician availability via Team Health
• Interdisciplinary team includes PT/OT, Dietician, Activities, Social Services, MDS/case managers, and Business Office biller
• Dress Code: Any color scrubs allowed
