Director of Care Coordination

Location: Greenbrae, California
Employment Type: Contract
Shift: Day Shift, Monday - Friday
Hours per Week: 40
Contract Length: 13 weeks

Required Qualifications

• Valid RN license in California
• Bachelor of Science in Nursing
• 5 years of manager experience
• Manager experience within the last year
• Experience in an acute care setting
• Case Management experience

Preferred Qualifications

• Master's degree in Nursing, Health Administration, or Business Administration
• Case Management certification such as CCM or ACM

Additional Information

• Temp Director of Care Coordination role
• Demonstrated experience leading interdisciplinary clinical/operational programs, staff, performance improvement, and regulatory or accreditation readiness
• Advanced knowledge of case management, care coordination, utilization management, discharge planning, patient progression, and transitions of care in an acute-care environment
• Up to 36 hours of call off available to the facility at no cost during the assignment
