Required Qualifications

  • Valid Case Management license in Vermont
  • Case Management (3 years of experience)
  • Case Management experience within the last year
  • BLS issued by American Heart Association
  • Master of Social Work (MSW) degree
  • Driver's License

  • Current skills checklist for Case Manager
  • One supervisory signed reference from within the past two years
  • Work history
  • Permanent address

Schedule / Assignment Notes

  • No schedule accommodations will be considered
  • Requested time off must be submitted at the time of application and is limited to 5 days or fewer
  • Traveler may not be working another perm placement or travel assignment concurrently

Work Setting / Role Focus

  • Coordinates interdisciplinary care across the care continuum
  • Supports patients with complex psychosocial, clinical, social, and environmental needs
  • Works with care transitions across settings such as home, clinic, skilled nursing, rehabilitation, and hospital
  • Utilization review experience is a plus

  • 8 hours of non-billable orientation
