Job Opportunity: Nurse Navigator (Registered Nurse)

Location: Orange, California

Work Schedule

• Shift: Days, Monday to Friday
• Hours: 8 hours/day, starting at 8:00 AM (varies based on clinic hours)

Required Qualifications

• License: Valid Registered Nurse (RN) license for California

Background and Health Requirements

• Criminal background check within 1 year, including statewide, nationwide, sex offender, SS trace, and federal searches
• Education verification and proof of graduation from accredited program
• Physical exam within 1 year, updated annually
• Negative 10-panel drug screen within 1 year, updated annually
• Immunizations and titers required:

  • TB test (negative PPD, TSpot, or QGold within 1 year)
  • Hepatitis B series or declination (titer not accepted)
  • Hepatitis C titer preferred; declination accepted with reason
  • MMR (Measles, Mumps, Rubella) positive titers or vaccinations
  • Varicella positive titer or vaccinations
  • Tetanus/Diphtheria/Tdap or Pertussis titer if unable to vaccinate
  • Seasonal Flu vaccine required by December 5 (declinations accepted with masking requirements)
  • COVID-19 vaccine documentation for current season or declination with masking requirements
• N95 mask fit testing within 1 year, renewed annually (PAPR acceptable with failed fit test proof)

Documentation and Compliance

• Signed HIPAA acknowledgment
• Signed facility-specific job description prior to start
• Signed orientation packets and acknowledgments (varies by role)
• Signed acknowledgment of pass/fail testing results (for Per Diem RNs)
• Signed acknowledgment of workplace violence prevention and sexual harassment policies
• References: 2 professional references within 1 year from leadership roles (e.g., Nurse Manager, Charge Nurse)

• Annual performance evaluation if employed for 1 year or more with Aya
