Job Description
- Role involves backfilling a Case Manager position for adult teams in a hospital setting.
- Case Managers are service-based and follow provider teams.
- Case Management Assistants support with CarePort referrals.
- Typical case load ratio: 1:28.
- Full-time floats are utilized for coverage during vacations, call-offs, PTO, and other scheduled days off.
- No weekends or holidays required.
- First-time travelers accepted.
Required Experience & Skills
- Minimum 2 years of care management experience with discharge planning in a hospital setting.
- Experience with CarePort (WellSky) and EPIC systems.
- Prior travel experience not required.
Certifications Required
- BLS (Basic Life Support) - AHA only, no certificates of completion accepted.
- ACLS (Advanced Cardiac Life Support) - AHA only, required as applicable.
- PALS (Pediatric Advanced Life Support) - AHA only, required for specific pediatric units (not required for UH PACU).
- NRP (Neonatal Resuscitation Procedure) - Required as applicable.
- Other certifications may be required depending on assignment.
Licensure & Verification
- State nursing license verification via primary source.
- NURSYS verification required for RN.
Background & Health Requirements
- 7-year background check within 30 days of start; must include all counties lived/worked, sex offender, nationwide.
- EPLS/GSA/SAM and HHS/OIG checks required prior to start and updated annually.
- Physical exam within 1 year of start; must be updated annually.
- Fit mask test within 1 year of start (N95 masks accepted).
- Negative TB test or chest X-ray within 1 year; if past positive, clear chest X-ray within 5 years and annual TB questionnaire required.
- Immunizations or titers required for: Rubeola (Measles), Mumps, Rubella, Hepatitis B, Varicella.
- Negative 10-panel drug screen within 30 days of start; instant drug screens not accepted.
- Seasonal flu vaccination required year-round with strict deadlines; no exemptions allowed.
Additional Requirements
- Government-issued photo ID required.
- I9 or attestation required.
- Completion of facility-specific orientation paperwork within 3 weeks of start.
- Professional references: 2 recent verified references from managers/supervisors (Charge RN references not accepted).
- Pass/fail competency testing (EKG/Medication) required for certain units (PACU, Tele, ICU including CVICU).
- Various facility-specific documents and attestations must be reviewed and signed prior to start (e.g., confidentiality agreement, code of conduct, sexual harassment policy).
Work Environment
- Service-based case management within a large hospital system.
- Coverage for adult teams.
- Support from Case Management Assistants.
- No weekend or holiday shifts required.
