Physical Therapist - Outpatient (Pelvic Floor, Pediatrics Preferred)

Job Description

• Pelvic Floor Physical Therapist with experience required.
• Pediatric experience preferred.
• No weekend shifts required.
• Orientation during the first few weeks in Boston and Waltham.
• Work schedule:

  • 2 days per week in Weymouth, MA
  • 3 days per week in Waltham, MA
• Shift: Day shift, 5 days a week, 8 hours per day (7:00 AM - 3:30 PM)

Required Qualifications

• Valid Physical Therapist license in Massachusetts

Compliance & Documentation Requirements

• Proof of name change if any documents are under a different name.
• Massachusetts Statewide iCORI release form required and must be run within 60 days of start date.
• Background check:

  • 10-year criminal background check within 6 months prior to start, covering all aliases and multiple jurisdictions.
  • EPLS/GSA/SAM and HHS/OIG checks required.
  • International background checks if candidate has lived/worked outside the U.S. in the last 10 years.
• Physical exam within 1 year of start, signed by MD, DO, PA, NP, or registered DC, stating cleared to work with no restrictions.
• Immunizations and health screenings required prior to start:

  • Tetanus/Diphtheria/Tdap (Tdap after age 11 acceptable, no declinations)
  • Negative TB test (blood or 2-step skin test) within required timeframes
  • MMR (Measles, Mumps, Rubella) proof of 2 vaccinations or positive titers
  • Hepatitis B documentation (series, titer, or declination)
  • Varicella proof of 2 vaccinations or positive titer
• Drug screening: 9-panel drug screen within last 12 months (not required for advanced practice nurses).
• Respiratory Safety clearance required for all travelers; facility-specific fit testing done onsite.
• Seasonal vaccinations and exemptions:

  • Flu vaccine or approved exemption required during flu season (8/1–3/31), with documentation due by 11/14.
  • COVID-19 initial vaccine series proof or approved exemption required; seasonal COVID vaccine preferred but not mandatory.
• Crisis Prevention Institute (CPI) certification required for behavioral health roles (not applicable here).

Orientation & Onboarding

• Orientation conducted in Boston and Waltham during the first few weeks.
• Candidates will receive an onboarding packet approximately 2 weeks prior to start, which must be completed and confirmed.

Additional Notes

• Candidates must complete facility-specific forms including data collection and background check attestations.
• Fingerprint-based background checks required for behavioral health roles (not applicable here).
• Candidates must submit required documentation timely to avoid delays or removal from assignment.

Summary

This is a 16-week travel contract for a Pelvic Floor Physical Therapist with pediatric experience preferred, working primarily weekdays with no weekends.
