- Day shift, Monday to Friday
- 8 hours per day (8:00 AM – 4:30 PM)
- No weekend shifts required
Unit Description
- Hospital-based outpatient clinic providing outpatient services and inpatient consults
Required Qualifications
- Valid LPN/LVN license in Massachusetts
- Minimum 2 years of LPN/LVN experience, including experience within the last year
- Prior travel nursing experience required
- Basic Life Support (BLS) certification
- Clinic experience
- Pass/fail testing on EKG and medication competency
Compliance and Documentation Requirements
- Valid government-issued ID
- Massachusetts Statewide (iCORI) Release (time sensitive)
- 7-year criminal background check within 30 days of start, including SSN trace, county searches, national criminal search, and federal exclusions
- Education verification of highest completed education (transcript acceptable if verification unavailable)
- Massachusetts Nurse Aide Registry Search (for tracking purposes)
- Physical exam within 12 months prior to start, including physician statement of fitness to work without restrictions
- Drug test within 30 days of start (specific drug panels required)
- Immunizations or valid declinations for:
- COVID-19 (most recent vaccine or facility-specific declination)
- Influenza (seasonal vaccine or declination during flu season)
- Tetanus/Diphtheria/Tdap (declinations not accepted for Pediatrics, NICU, Maternity)
- MMR (Measles, Mumps, Rubella) with titers, vaccines, or declinations (declinations not accepted for Pediatrics, NICU, Maternity)
- Varicella (titer, vaccine, documented history, or declination)
- Hepatitis B (titer, 3-dose series, or declination)
- Tuberculosis screening:
- Two PPD tests (one within 90 days, one within 1 year) or QuantiFERON/T-Spot within 90 days
- If positive, chest X-ray within 5 years and facility-specific TB questionnaire with physician clearance
- Pass/fail medication competency exam required on first day of orientation (minimum 80% score)
- NURSYS license verification within 30 days prior to start
- Employment application covering 7 years of employment history with explanations for gaps over 30 days
- Two professional references within the last three years
- Skills checklist completed prior to start and updated annually as applicable
Additional Notes
- Candidates who have worked in the same hospital system within the last 12 months are preferred
- Compliance documents must be current and uploaded according to specified expiration guidelines
- Failure to pass required competency exams on orientation day may result in immediate cancellation of assignment
