Physical Therapist Home Health - Travel/Contract

Location: Marlborough, Massachusetts
Contract Length: 13 weeks

Job Overview

• Provide physical therapy services in a home health setting with patient home visits.
• Average caseload: 5-6 patients per day, typically 5-10 patients assigned daily.
• Types of visits: Start of Care and Follow Ups.
• Schedule: Monday to Friday, 8:00 AM to 5:00 PM (5x8 hours).
• Travel: Must be willing to travel up to 75 miles from the home clinic, averaging 100-150 miles per week. Mileage reimbursed at IRS standard rate (patient home to patient home).
• EMR system used: HCHB.
• Weekend and on-call requirements vary by location; discuss during interview.

Required Qualifications

• Minimum 1 full year of recent Adult Home Health physical therapy experience (first-time home health therapists not accepted).
• Current Physical Therapist license valid in Massachusetts (license must be in hand; pending licenses accepted only if issuance is imminent).
• CPR and BLS certification (AHA, National CPR Foundation, or ARC accepted).
• Reliable transportation and a current driver’s license.
• Agency-required liability insurance.

Compliance and Documentation Requirements

• Must complete a standard background check covering the last 7 years, including SS trace, county, nationwide, sex offender, and state-specific searches.
• Required to complete Massachusetts Statewide iCORI background check and CORI release authorization.
• Drug screening: 10-panel drug test required.
• Health documents:

  • Physical exam within 12 months confirming good health and no communicable diseases.
  • Negative TB test or chest X-ray within 12 months (no pending results at start).
  • Proof of immunizations or titers for Measles (Rubeola), Mumps, Rubella, Hepatitis B, Varicella.
  • Seasonal flu vaccination or documented declination.
  • COVID-19 vaccination per Massachusetts state policy (full vaccination required; no declinations accepted).
• Education verification for applicable degree.
• Verification of at least two most recent employers or last 7 years of work history.
• Two professional references required.

• Must review and sign various compliance documents prior to start, including but not limited to:

  • Compliance Training Certification
  • Home Health Core Policy Acknowledgement and Scenarios
  • COVID-19 Policy and Attestation
  • Employee Responsibility Agreement
  • TB Questionnaire
• Required to sign new supplier packets at the start of each assignment and annually if working more than one consecutive year.

• No guaranteed hours; if patient census is low, options to float to nearby clinics may be discussed.
