Location: Medford, Oregon
Employment Type: Travel/Contract
Contract Length: 13 weeks

Job Details

Shift: Day shift, 8 hours per day (8:00 AM - 4:30 PM), 5 days a week (40 hours/week)
Schedule: 8-hour days only; no 4x10 or 3x12 shifts available
Patient Load: 1:1 ratio, may visit up to 6 patients per day (30 productivity points standard)
Start Date: ASAP
Weekend Requirement: No
On-Call: No
Floating: No floating between units required
Block Schedule: Not accommodated
Locals Accepted: Yes, within 50-75 mile radius
Transportation: Traveler must have own transportation; occasional travel up to 50+ miles
Guaranteed Hours: Facility may call off up to one shift every two weeks

Required Qualifications

Valid Physical Therapist license in Oregon (pending licenses accepted)
Minimum 1 year of Physical Therapist experience within the last year
Experience in a Home Health setting
Experience with OASIS documentation
Basic Life Support (BLS) certification

Compliance and Background Requirements

Government-issued photo ID (Driver’s License or State ID)
Social Security Card (as applicable)
Background check within 90 days covering all locations lived/worked in past 7 years, including:
- SSN Trace, Death Master Index, Counties, Nationwide, National Sex Offender, FACIS Level 3, Education Verification, Motor Vehicle Record (MVR)
State-specific searches as required
E-Verify attestation
Education verification (degree/diploma/transcript as applicable)
Drug screening: 9-panel within 12 months of start
Physical exam within 1 year of start, signed by MD, NP, PA, or DO, indicating ability to work with or without accommodation
Respirator clearance (RespSafety) required annually for travel candidates
Annual flu vaccination or signed declination during flu season
TB test or chest X-ray within 12 months, updated annually
Other immunizations: TDAP (within 10 years), MMR (measles, mumps, rubella), Varicella, Hepatitis B (vaccination or declination)

Facility groups clients to minimize drive time, but occasional long drives (50+ miles) may be required
