Employment Type: Travel/Contract
Contract Length: 13 weeks
Job Details
- Shift: Day shift, 8 hours per day (8:00 AM - 4:30 PM), 5 days a week (40 hours/week)
- Schedule: 8-hour days only; no 4x10 or 3x12 shifts available
- Patient Load: 1:1 ratio, may visit up to 6 patients per day (30 productivity points standard)
- Start Date: ASAP
- Weekend Requirement: No
- On-Call: No
- Floating: No floating between units required
- Block Schedule: Not accommodated
- Locals Accepted: Yes, within 50-75 mile radius
- Transportation: Traveler must have own transportation; occasional travel up to 50+ miles
- Guaranteed Hours: Facility may call off up to one shift every two weeks
Required Qualifications
- Valid Physical Therapist license in Oregon (pending licenses accepted)
- Minimum 1 year of Physical Therapist experience within the last year
- Experience in a Home Health setting
- Experience with OASIS documentation
- Basic Life Support (BLS) certification
Compliance and Background Requirements
- Government-issued photo ID (Driver’s License or State ID)
- Social Security Card (as applicable)
- Background check within 90 days covering all locations lived/worked in past 7 years, including:
- SSN Trace, Death Master Index, Counties, Nationwide, National Sex Offender, FACIS Level 3, Education Verification, Motor Vehicle Record (MVR)
- State-specific searches as required
- E-Verify attestation
- Education verification (degree/diploma/transcript as applicable)
- Drug screening: 9-panel within 12 months of start
- Physical exam within 1 year of start, signed by MD, NP, PA, or DO, indicating ability to work with or without accommodation
- Respirator clearance (RespSafety) required annually for travel candidates
- Annual flu vaccination or signed declination during flu season
- TB test or chest X-ray within 12 months, updated annually
- Other immunizations: TDAP (within 10 years), MMR (measles, mumps, rubella), Varicella, Hepatitis B (vaccination or declination)

References and Employment History
- Minimum 2 references covering at least 1 year of employment within the last 3 years in a comparable specialty/setting
- Employment application covering at least last 5 years of employment with month/year format for dates worked
- Work history verification may be required if reference requirements are not met

- Pending license holders are accepted but must confirm with hiring manager regarding return-to-office restrictions
- Facility groups clients to minimize drive time, but occasional long drives (50+ miles) may be required
- No weekend or on-call shifts required
