• Approximately 13 weeks

Work Schedule

• Day shift, 8 hours per day (8:00 AM – 4:30 PM)
• 5 days per week (40 hours weekly)
• Guaranteed 36 hours weekly after initial ramp-up period
• Weekend patient visits possible to meet productivity goals
• No on-call required

Job Description

• Home Health Physical Therapist Assistant (PTA)
• Patient ratio typically 1:1, may see up to 5-6 patients per day
• Standard productivity expectation: 27 productivity points daily
• Travel radius may occasionally exceed 50 miles; own transportation required
• System groups clients to minimize drive time
• Timely communication and documentation required; documentation tools provided

Required Qualifications

• Valid Physical Therapy Assistant license in California
• Minimum 1 year of Physical Therapy Assistant experience within the last year
• Experience in a Home Health setting
• Basic Life Support (BLS) certification
• Must have reliable personal transportation

Additional Requirements & Compliance

• Government-issued photo ID (Driver’s License/State ID)
• Emergency contact form prior to start
• Background checks including 7-year history and national registries
• Annual EPLS/GSA/SAM and HHS/OIG checks
• Physical exam within 1 year confirming ability to work without restrictions
• Immunizations/vaccinations or documented immunity for:

  • Tetanus/Diphtheria/Tdap (within 10 years or annual declination)
  • Measles, Mumps, Rubella (2 vaccines or positive titers within required timeframes)
  • Hepatitis B (titer, vaccine series, or declination)
  • Varicella (2 vaccines or positive titer)
  • Flu vaccine or declination (year-round requirement)
  • COVID-19 vaccine or declination form (religious/medical exemptions accepted)
• Negative TB test or chest X-ray within 1 year
• 10-panel drug screen within 1 year
• Auto insurance and current vehicle registration required
• Completion of facility-specific forms including arbitration and confidentiality agreements
• Professional references and work history verification related to profession required

• Mileage reimbursed at IRS standard rates
