Job Opportunity: Physical Therapist Assistant (PTA) - Outpatient

  • Monday to Friday, 8:30 AM to 5:00 PM (40 hours per week)

Job Description

  • Position: Physical Therapist Assistant (PTA) in Home Health setting
  • Home Health experience is preferred; all PTA experience will be considered
  • Must have Home Health experience to qualify

Required Qualifications

  • Valid Physical Therapy Assistant license in South Carolina (pending state license accepted but must be obtained prior to starting)
  • Physical Therapy Assistant with at least 1 year of experience within the last year
  • Experience in a Home Health setting
  • Basic Life Support (BLS) certification (AHA or RQI acceptable)
  • Must carry government-issued photo ID during hours of service
  • Attendance policy: Missing more than two shifts in a 13-week contract without reasonable explanation may lead to contract review and possible cancellation
  • State license primary source verification required
  • Orientation Safety Manual Acknowledgement required
  • Background checks including 7-year criminal history, EPLS/GSA/SAM, HHS/OIG within 30 days of start and updated annually
  • Education verification required (highest level of education must be associate degree or higher; certificates alone are not sufficient)
  • Drug screening within 90 days of start (10 panel plus Oxycodone and Marijuana)
  • Physical exam within 1 year of start, updated annually
  • Fit mask (N95) fit test within 1 year, updated annually
  • TB risk assessment and symptom evaluation within 3 months of start, updated annually; negative TB test or chest X-ray documentation required as applicable
  • Immunizations/vaccinations required or approved exemptions for:
    • Measles (Rubeola)
    • Mumps
    • Rubella
    • Varicella
    • Hepatitis B (3-dose series, 2-dose Heplisav, positive titer, or signed declination)
    • Seasonal Flu vaccination or declination (mask required if declined during flu season)
    • COVID-19 doses 1 and 2 required or declination form
  • References: 2 references within 1 year or 3 years depending on facility (Qualivis requirement)
  • Employment application required
  • Skills checklist updated annually for applicable units
  • Must submit all requested Return to Office (RTO) restrictions with initial submission; late requests will not be considered
  • Facility-specific documents including photo for badge and compliance checklists required
  • South Carolina SLED criminal records search required regardless of residency history
