ARRT (Radiation Therapy) certification
BLS (Basic Life Support) certification
Additional certifications may be required based on clinical track (verify individually)

Government-issued photo ID (Driver’s License or State ID)
Social Security Card (as applicable)
Background check within 90 days covering all locations lived/worked in past 7 years, updated annually
Education verification (degree/diploma/transcripts as applicable)
Physical exam within 1 year of start, signed by MD, NP, PA, or DO
Immunizations and health screenings including:
- Negative TB test or chest X-ray within 12 months
- TDAP vaccine within 10 years (declination accepted)
- MMR (Measles, Mumps, Rubella) vaccination or positive titer
- Hepatitis B vaccination or signed declination
- Varicella vaccination or positive titer
- Seasonal flu vaccine during flu season (declination accepted)
- COVID vaccination record preferred (declinations not required)
Drug screening: 9-panel drug screen within 12 months (instant acceptable with attestation)
Respiratory safety clearance for respirator use (required annually for travel candidates)

Minimum of 2 professional references covering at least 1 year of employment in a comparable specialty/setting within the last 3 years
Employment application covering at least the last 5 years of employment with explanations for any gaps or overlaps
Annual completion of regulatory and specialty-specific training modules via LMS (Workramp)
Passport-style badge photo with neutral background required

Valid Radiation Therapist license in California
Minimum of 1 year of Radiation Therapist experience, including experience within the last year
ARRT certification in Radiation Therapy (ARRT (T))
Basic Life Support (BLS) certification
Experience as a CT Technologist or with CT technology
Current State License for Radiation Therapy
