Job Opportunity: Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN) - Travel/Contract
Location

Job Description
- Provide and coordinate home nursing care according to physician orders and agency policies.
- Directly supervise and evaluate Home Health Aides and other LVNs/LVNs.
- Conduct home visits with an average patient load of 5-8 patients per day.
- Types of cases include Start of Care, Wound Care, Follow-up Visits, etc.
- Must have reliable transportation, a current driver’s license, and required liability insurance.
- Travel radius up to 75 miles from the home clinic.
- Work schedule typically Monday through Friday, 8 AM to 5 PM.
- No guaranteed hours; patient census may vary.
- Use of HomeCare HomeBase (HCHB) electronic medical record system required.
- Dress code: Scrubs.
Contract Length
- 13 weeks
Required Experience
- Minimum 1 full year of recent Adult Home Health experience or 1 full year Hospice experience depending on specialty.
- Recent specialty experience required; first-time travelers are not accepted.
Required Certifications and Licenses
- Current LVN/LPN license in Oregon (license must be in hand; pending licenses not accepted).
- CPR and Basic Life Support (BLS) certification required.
Work Environment and Expectations
- Home health setting.
- Must be proficient with HomeCare HomeBase (HCHB) EMR; retraining required if not used within the last 6 months.
- Weekend and on-call requirements vary by location and should be discussed during the interview.
- Mileage reimbursed at IRS standard rate for travel between patient homes (commute to/from first and last patient not reimbursed).
Required Qualifications
- Valid LPN/LVN license in Oregon
- Minimum 1 year of experience in Home Health, including experience within the last year
- Experience working with adult patients in a Home Health setting
- Hospice experience
- BLS certification issued by the American Heart Association or American Red Cross
- Proficiency with HomeCare HomeBase (HCHB) electronic medical record system
Prior Travel Experience
- Not explicitly required, but recent specialty experience is mandatory; first-time travelers are not accepted.
Additional Requirements
- Must complete standard background checks, drug screening, and health documentation including:
- Physical exam within 12 months.
- Negative TB test or chest X-ray within 12 months.
- Immunizations or titers for Measles, Mumps, Rubella, Hepatitis B, Varicella.
- COVID-19 vaccination or declination.
- 10-panel drug screen.
- Must review and sign various compliance, confidentiality, and policy documents prior to start.
- Must have personal or auto insurance policy covering home health visits.
Travel and Mileage
- Expected to travel approximately 150 miles per week within the assigned territory.
- Local candidates accepted.
