Provide physical therapy services in an inpatient rehabilitation setting.

Proof of I9 or attestation.
Government-issued photo ID (must be carried during hours of service).
Proof of E-Verify or attestation.
Background check: 7-year criminal history check covering all locations lived/worked in the past 7 years, including SS trace, counties, nationwide, sex offender registry, and FACIS level 3.
EPLS/GSA/SAM and HHS/OIG checks required within 30 days of start and updated for every contract renewal/extension.
Primary source verification of state license.
Medication/Pharmacology competency exam as applicable.
Clinical references: proof of 2 clinical references or hospital evaluations.
Employment application/resume.
Skills checklist updated annually for applicable unit.
Performance evaluation if worked over 1 year; internal evaluation if over 2 years with the agency.

Initial physical exam and annual updates attesting ability to meet physical demands.
Negative TB test (PPD, Quantiferon Gold, or T-spot) within 1 year, updated annually; if past positive, clear chest X-ray within 1 year and annual TB questionnaire.
Immunizations or titers for:
- Rubeola (Measles)
- Mumps
- Rubella
- Hepatitis B (titer, vaccine series, or declination)
- Varicella
9-panel drug screen within the last 12 months (instant acceptable).
