- Immunizations and Titers:
- Tetanus/Diphtheria/Tdap (vaccine or titer, declination accepted with form)
- Negative TB test or chest X-ray within 1 year
- Measles (Rubeola), Mumps, Rubella: Titer or 2 vaccines required, no declinations allowed
- Hepatitis B: Vaccine, titer, or declination form required
- Varicella: Titer or 2 vaccines required, declinations accepted only for medical/religious reasons
- Flu Vaccine: Required annually, exemptions accepted with approval and proper forms
- COVID-19: Full vaccination required prior to start; booster preferred but not required; medical/religious exemptions subject to approval
- Drug Testing: 9-panel drug screen within last 12 months (instant acceptable)
- References: Proof of 2 clinical references or hospital evaluations
- Employment Application: Complete with licensure, certifications, education, and employment history
- Skills Checklist and Competency Exams as applicable

Requirements/Qualifications
- Valid Registered Nurse (RN) license in the District of Columbia
- Minimum 2 years of Surgical Intensive Care Unit (SICU) nursing experience, with experience within the last year
- ACLS certification issued by American Red Cross or American Heart Association
- Basic Life Support (BLS) certification
- NIH Stroke Scale (NIHSS) certification
- Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
- At least 2 years of SICU RN experience (use 2 years as the lower bound)
