Job Description
- Work schedule: 5x8 rotating shifts with every third weekend coverage
- First-time travelers not accepted
- Required skills:
- Inpatient/Acute care pharmacy
- Pharmacokinetic dosing
- Renal dosing
- Therapeutic medication monitoring
- Drug shortage management
- Pharmacotherapy management for medical problems
- Epic experience required
- Scrubs provided
- Travelers must shave for mask fit testing (no exceptions; CAPRs and PAPRs not provided or allowed)
Required Qualifications
- Valid Clinical Pharmacist license in Wisconsin
- Doctor of Pharmacy (PharmD) degree
- Minimum 2 years of clinical pharmacist experience, including experience within the last year
- Completion of at least one travel assignment
- Proficient in pharmacokinetic dosing, renal dosing, therapeutic medication monitoring, and drug shortage management
- Experience with Epic electronic health record system
Prior Travel Experience
- Prior travel experience required (first-time travelers not accepted)
Compliance & Documentation Requirements
- Government-issued photo ID and valid Wisconsin pharmacy license
- Background check within 12 months including SSN trace, 7-year county residence/work history, nationwide search, sex offender search
- Fit testing for respirator mask required onsite during first week of orientation
- OSHA Respirator Clearance required prior to start
- Negative drug screen within 12 months of start
- Immunizations and health documents required:
- Flu vaccine or approved medical/religious exemption (annual renewal required)
- TB test (PPD, Qgold, or TSPOT) within 12 months
- COVID vaccine series preferred
- Hepatitis B titer, series, or declination (for new candidates)
- Measles (Rubeola), Mumps, Rubella titers or vaccination proof
- Varicella titer or vaccination proof
- Physical exam within 1 year indicating good health
- Completion of facility-specific orientation modules and documents prior to or during orientation
- Skills checklist and references required
Additional Notes
- Candidates must complete all required modules assigned for their unit annually
- Post-start orientation checklists must be completed and submitted within the first week
- Failure to comply with fit testing or documentation requirements may result in assignment cancellation
This position is ideal for experienced inpatient pharmacists with prior travel contract experience who meet Wisconsin licensure and certification requirements and are comfortable with rotating shifts and strict compliance protocols.
