Required Qualifications

• RN license: compact or Ohio state license; temporary licenses are not accepted
• BLS certification from the American Heart Association
• 2 professional references from the last 2 years
• Resume including work history and education
• Job application completed by the candidate
• Prior employment verification within the last 5 years
• Education verification
• Skills checklist
• Competency exam with a passing score of at least 80%
• Core competencies with a passing score of at least 80%
• Physical exam statement showing ability to work without restrictions
• U.S. I-9 attestation or completed I-9 on file
• Ohio fingerprints and criminal background check
• Criminal background check
• OIG search
• SAM/OFAC screening
• Drug screen
• Background check within 6 months of start
• Module completion certificate
• Badge photo
• License/certificate verification
• MMR immunity or vaccination documentation
• Varicella immunity or vaccination documentation
• Hepatitis B immunity, vaccination series, or signed declination
• Tdap vaccination within the last 10 years
• TB test within the last 6 months
• Fit test within 1 year prior to start
• Influenza vaccine documentation or approved exemption paperwork
• Prior employment form

Additional Notes

• Proof of flu vaccine is required from October through March
• If declining influenza vaccination, masking is required while working
• COVID-19 vaccination is preferred but not required
