Job Description

• Position in a Level III Trauma Center.
• Schedule: Four 10-hour shifts per week (07:00 - 17:30), Monday through Friday.
• On-call requirement: Minimum 1-2 weekdays per week and 1-2 weekend 24-hour call shifts per month (Saturday or Sunday).

Required Qualifications

• Valid Registered Nurse (RN) license in Ohio
• Minimum 1 year of Operating Room (OR) experience, with OR experience within the last year
• BLS certification issued by the American Heart Association
• ACLS certification issued by the American Heart Association
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)

Health and Safety Requirements

• COVID-19 Vaccine: Preferred but not required; medical or religious exemptions accepted with approval.
• MMR: Positive titers within last 15 years required; booster protocol applies if low titer.
• Varicella: Positive titers within last 15 years required; booster protocol applies if low titer.
• Tdap: Vaccination within last 10 years required; no declinations accepted.
• Hepatitis B: Proof of full series or partial series with signed declination accepted; positive titers within 15 years accepted.
• Influenza Vaccine: Required during flu season (October-March) or mask-wearing and specific declination forms required.
• Tuberculosis Testing: 2-step PPD or IGRA within last 6 months; if positive, clear chest X-ray and current TB questionnaire required.
• Fit Test: Completed within 1 year prior to start; annual update required.
• Physical Exam: Pre-employment physical exam within 1 year confirming ability to work without restrictions.

Background and Compliance

• Criminal Background Check: Within 6 months of start, including Social Security trace, county searches, national sex offender registry.
• Ohio Fingerprints: Required for Ohio residents; includes BCI&I and FBI checks depending on residency duration.
• Employment Verification: Verification of employment within last 5 years required.
• Education Verification: Highest level of education verified through third-party.
• I-9 Attestation: Proof of eligibility to work required.
• OIG Exclusion Check: Must be completed within 6 months prior to start.
• Drug Screen: 10-panel drug screen within 60 days prior to start; instant drug screens not accepted.
• Social Security Trace and National Database Criminal Background Check may be required.

Skills and Competency

• Skills Checklist: Must be completed within 1 year prior to start (clinical skills specific).
• Competency Exam: Must score at least 80%; completed within 1 year prior to start.
• Core Competencies: Must score at least 80%; includes HIPAA, OSHA, Joint Commission topics; completed within 1 year prior to start.
• Safety and Compliance Training: Completion of required safety modules prior to clearance; annual renewal required.
