Work Schedule & Conditions

• Every third weekend required
• One holiday shift if a holiday occurs during the contract
• No Return to Origin (RTO) approved in March

Required Credentials & Certifications

• BLS (Basic Life Support) – American Heart Association certification required for all direct patient care personnel
• ACLS (Advanced Cardiovascular Life Support) – Required for RNs in critical care areas

• Influenza Vaccine – Proof of vaccine, declination, or exemption required for flu season
• 2 Step PPD OR IGRA (Quantiferon/Tspot) – TB testing required; chest x-ray if positive
• Varicella Vaccine – Proof of vaccination or IgG titer required
• Physical/Health Statement – Documentation of fitness to work within the last year
• 10 Panel Drug Screen – Completed within 30 days of start, no THC allowed
• Trinity Profile – Resume, skills checklist within a year, and 2 professional references required for submission
• Conflict of Interest Disclosure Form
• Criminal Background Check – Federal, nationwide, county criminal checks from last 7 years, including SSN verification
• Education Verification – Highest level of academic education verified
• EPLS (Excluded Parties List System) Check
