Physical Therapist (Pediatrics) - Therapy Annex

Job Description
- Perform patient transfers with assistance as needed from bed, chair, and floor.
- Use safe and appropriate body mechanics to reduce injury risk to self, staff, and patients.
- Treat patients and colleagues with dignity, respect, empathy, and care.
- Design and adjust plans of care integrating goals, treatment, outcomes, and discharge plans based on patient response and evidence.
- Educate patients, families, caregivers, students, and healthcare providers using effective teaching methods.
- Evaluate clinical findings to determine physical therapy diagnoses and outcomes.
- Perform examinations using reliable and valid methods relevant to the chief complaint, screening results, and history.
- Produce documentation in accordance with clinical process policies.
- Screen patients to determine the need for therapy services.
- Utilize support personnel according to legal and ethical guidelines; monitor and provide feedback on their care delivery.
- Complete annual education as required for compliance with DNV Hip and Knee Replacement Program (if applicable).
- Evaluate impairments related to orthopedic and neurologic conditions.
- Establish plans of care based on functional deficits and patient goals.
- Conduct objective testing for range of motion, strength, and daily functional tasks.
- Complete daily documentation and charging in therapy EMR.
- Apply tape for stabilization, facilitation, or inhibition of movement.
- Administer therapeutic modalities such as ultrasound, interferential, and iontophoresis.
- Communicate with referring providers regarding patient progress and treatment recommendations.

- Education: Bachelor's degree in Physical Therapy
- Certifications/Licensure:
- BLS (American Heart Association)
- Licensed Physical Therapist (PT) - Illinois Department of Financial and Professional Regulation (IDFPR)
