Job Description

• Conduct initial assessments for patients on legal holds related to psychiatric care.
• Complete social determinants of health evaluations as part of assessments.
• Evaluate patients’ mental status, living situation, support system, substance use history, mental health history, physical functioning, financial status, and discharge planning needs.
• Communicate regularly with nursing management and unit social workers/case managers regarding patients on legal holds.
• Maintain contact with external agencies and families for collateral information and discharge arrangements (non-medical).
• Monitor patients daily or multiple times per week depending on legal hold status.
• Assist with discharge planning, including transfers to approved facilities or alternative plans when beds are unavailable.
• Monitor conservatorship potential for patients with grave disability status and coordinate timely referrals.
• Collaborate with psychiatrists and medical social workers regarding referrals and patient care transitions.

Required Qualifications

• Valid Licensed Clinical Social Worker (LCSW) license in California.
• Minimum of 1 year of experience as a Licensed Clinical Social Worker.
• Licensed Clinical Social Worker experience within the last year.

Additional Information

• Must be able to type at least 30 words per minute without errors.
• Must be able to distinguish colors and sounds as necessary for the position.
