Required Certifications & Training

• BLS (American Heart Association certification required for all direct patient care personnel)
• ETW/ETH (as per job description)
• Trinity Child and Adult Dependent Abuse Training (mandatory for onboarding)
• Trinity Skills Checklist (provided by agency for clinical contractors)
• Specialty exam competency testing (must be completed within the last year)

Licenses & Verifications

• Valid license/certificate verification dated within 30 days of start
• Drug Screen: 9 Panel (completed and resulted before start, no THC)
• Fit Test (required for onboarding)
• Criminal Background Check (Federal, Nationwide, County for last 7 years, dated within 30 days of start)
• Education Verification (highest level of academic education verified)
• EPLS and OIG Exclusion checks (dated within past 30 days)
• NSO (National Sex Offender) check dated within 30 days of start
• OFAC Sanctions Search (must be completed within 30 days of start)

Health & Immunization Requirements

• COVID-19 Vaccine: Proof of booster, declination, or exemption (declination not accepted in CA or IL)
• MMR: Proof of 2 vaccinations or IgG titer lab report
• Hepatitis B: Proof of vaccination series (titers not accepted)
• Influenza Vaccine: Recommended (optional)
• Varicella: Proof of 1 vaccination or IgG titer lab report
• 2 Step PPD OR IGRA (Quantiferon/Tspot) test
• Physical/Health Statement: Documentation from exam within last year confirming fitness to work

Compliance & Documentation

• Trinity Code of Conduct Form (signed and on file)
• Trinity Confidentiality of Patient & Proprietary Information and System Security Agreement (signed and on file)
• Trinity Integrity and Audit Services Form (signed prior to start)
• Trinity Non Employee Upload Form (pages 1-3 and 28 required)
• Trinity Profile: Resume, Skills Checklist (within a year), and 2 Professional References (required for submission)
• Trinity Conflict of Interest Disclosure Form (signed and on file)
• Trinity I9 form (completed by agency)
• Trinity System Access Form (completed and uploaded one week before start)
• Trinity Specific COVID Questionnaire (completed and uploaded)
• Trinity Joint Commission Testing (CORE Exam within a year)

• Iowa state-specific requirements apply (e.g., criminal history and abuse registry checks must be done within 30 days prior to start)
