Job Description

• 36 hours per week full-time, but open to any hours
• EMR access required
• Fingerprinting required
• Basic Life Support (BLS) certification required

Required Qualifications

• License: Valid LPN license for Iowa
• Certifications:

  • American Heart Association BLS certification (copy of front and back of card required)
  • NIH Stroke Scale (NIHSS) certification
• Health Requirements:

  • 9 or 10 panel drug screen completed within 30 days of start (no THC)
  • COVID-19 vaccine or approved exemption/declination (with agency letterhead)
  • MMR vaccination or immunity proof
  • Hepatitis B vaccination series (proof required; titers not accepted)
  • Influenza vaccine or approved exemption/declination (for current flu season)
  • Varicella vaccination or immunity proof
  • Tdap vaccine (declinations accepted if no pediatric interaction)
  • 2 Step PPD or IGRA (Quantiferon/Tspot) test with chest x-ray if positive (per regional requirements)
  • Physical/Health statement confirming fitness to work (within last year)

Background and Compliance Checks

• Federal, nationwide, and county criminal background check (last 7 years) including SSN verification (within 30 days of start)
• Sex offender registry check (NSO) within 30 days of start
• Iowa-specific criminal history, sex offender, dependent adult & child abuse registry check (within 30 days of start)
• OFAC sanctions check (within 30 days of start)
• EPLS (Excluded Parties List System) check (within 30 days of start)
• OIG/GSA exclusion check (within 30 days of start)

Additional Requirements

• Fit test for respirators
