Job Opportunity: RN Critical Care

Location: Lafayette, Louisiana
Employment Type: Long-Term Assignment (LTA)
Contract Length: 12 weeks

Required Specialty and Experience

• Specialty: Registered Nurse (RN) - Critical Care

Required Credentials and Certifications

• Licenses:

  • Active professional nursing license verified by primary source within 30 days of start
  • Nursys verification required within 30 days of start

• Certifications:

  • BLS (AHA or ARC only)
  • ACLS (AHA only)

• Professional Documentation:

  • Resume/Profile required for submission
  • Criminal Background Check (completed within 30 days prior to start) including:

    • SSN Address Trace report (7 years)
    • County criminal searches for all counties lived/worked in last 7 years
    • National criminal check including NSO, OFAC, OIG, and SAM
    • Education verification (3rd party)
    • Employment verification (last 5 years)
    • Driving record (if applicable)
  • HRIS Form required for submission
  • Badge photo (forward facing, well-lit, neutral background, JPEG)

Health and Immunization Requirements

• COVID-19 Vaccine or approved exemption accepted
• MMR: Proof of 2 immunizations, positive titer, or declination with signed statement
• Hepatitis B: Proof of 3 immunizations, positive titer, or declination with required approvals
• Influenza Vaccine: Required during flu season (10/1 - 4/30), medical or religious exemptions accepted with proper documentation
• TB Test: Negative QuantiFERON blood test within 1 year or 2-step skin test; if positive, must provide MD clearance or proof of treatment
• Varicella: Proof of 2 immunizations, positive titer, or declination with signed statement
• Tdap: Proof of vaccination within past 10 years or declination with required approvals
• Annual TB Questionnaire: Required within 6 months of start; if positive, documentation of evaluation and treatment completion required
• Health Statement: Signed and dated within 1 year confirming ability to work without restrictions
• Eye Exam & Color Vision: Ishihara Color Test and Snellen Vision test within 1 year prior to start

Testing and Competency Requirements

• Drug Screen: Negative 10-panel drug screen within 30 days prior to start (includes THC, PCP, amphetamines, opiates, cocaine, benzodiazepines, barbiturates, methadone, propoxyphene, methaqualone)
• Skills Checklist: Unit-specific checklist within 1 year prior to start, updated annually
• Competency Exam: Passing score of 80% or higher within 1 year prior to start
• Core Competencies: Must be current within 1 year prior to start with passing score of 80% or higher; includes HIPAA, Patient Bill of Rights, Safety Orientation, Bloodborne Pathogens, Fire Safety, Infection Control, Elder Abuse and Neglect, Age-Specific Care, Cultural Diversity, Advance Directives; renewed annually
• Contract Annual Education Packet: Required

This position requires a critical care RN with all standard licensure, certifications, health screenings, and competency validations up to date and verified prior to start. The assignment is a 12-week long-term contract in Lafayette, Louisiana.
