Job Description

• Home Health RN position in Southern California, Ventura County
• Varied work days: Tuesday-Saturday or Wednesday-Sunday
• Requires 1 year of home health experience, specifically with geriatric patients
• BLS/CPR certification required
• California nursing license preferred; pending CA license accepted if application submitted
• OASIS experience required
• Guaranteed hours based on completed visits (25 visits = 40 hours)
• COVID exemptions permitted
• Mileage reimbursed at IRS standard rates

Required Certifications & Licenses

• BLS (Basic Life Support) - AHA Instructor or Provider acceptable
• CA Nursing License (preferred or pending with application)
• OASIS experience
• Other certifications as applicable (ACLS, PALS, NRP, EFM) - verify based on assignment

Required Documents & Compliance

• Government-issued photo ID (Driver’s License or State ID)
• Valid passport copy before start
• Background checks including 7-year history, EPLS/GSA/SAM, HHS/OIG (annual)
• State license verification and NURSYS verification (for RN, LPN/LVN, AP)
• Medication/Pharmacology competency exams as applicable (minimum 80% score)
• Photo for badge (passport style)
• Personal or auto insurance with current expiration dates
• Auto registration documentation
• Facility-specific training and policy acknowledgments (e.g., hand hygiene)
• Physical exam within 12 months of start
• Immunizations and health screenings:

  • Tdap vaccine within past 10 years (no declinations)
  • Negative TB test or chest X-ray within 1 year (with protocols for past positives)
  • Measles, Mumps, Rubella (MMR) proof or titers for candidates born 1957 or later (no declinations)
  • Hepatitis B titer or vaccine series (declination accepted if within 1 year)
  • Varicella immunity proof (no declinations)
  • Negative 9-panel drug screen within 1 year
  • Seasonal flu vaccination/documentation (declinations accepted)
  • COVID-19 vaccination or approved medical/religious exemption required prior to start, including booster dose
