Job Opportunity: PT Home Health (Therapy/Rehabilitation)

Location: Youngstown, Ohio (Columbiana, Mahoning, and Trumbull Counties)
Employment Type: Travel/Contract
Contract Length: 13 weeks

Job Description

• Specialty: Home Health Physical Therapy
• Average Patients per Day: 5-7 (to be confirmed during interview)
• Productivity Expectation: 6 points per day / 30 points per week
• Guaranteed Hours: 40 hours per week
• Types of Cases: Geriatric
• Territory: Up to a 60-mile radius from the hub/office
• Dress Code: Scrubs (any color), no jeans
• Equipment Provided: iPad
• Mileage Reimbursed at IRS standard rate
• Cell phone use: Not allowed
• Flexibility with territory expected
• Timely documentation and communication required
• Locals accepted at travel rate
• First-time travelers NOT accepted

Required Qualifications

• Valid Physical Therapist license in Ohio
• Minimum 1 year of Physical Therapist experience within the last year
• BLS (Basic Life Support) certification
• Experience working with Geriatric patients
• Experience in a Home Health setting
• Experience with Homecare Homebase (HCHB) EMR

Compliance and Background Requirements

• Background checks required within 90 days of start, including:

  • SSN Trace
  • County searches for last 7 years of residence/work/school
  • FACIS Level 1M
  • Nationwide Sex Offender
  • Motor Vehicle Records (MVR)
  • Education and Work History Verification
  • State-specific searches as applicable (e.g., Kentucky, Virginia, Indiana)
• EPLS/GSA/SAM and HHS/OIG checks required within 90 days of start
• Fingerprinting: Ohio BCI and FBI fingerprint results required within 12 months of start and updated annually
• Education verification of highest degree related to profession
• NURSYS verification (if applicable)
• Drug Testing: 9 Panel Drug Screen within last 12 months
• Physical Exam: Physician statement within last 12 months
• Immunizations and Health Documents:

  • Negative TB test or chest X-ray within required timeframes
  • TB Questionnaire (annual)
  • Rubeola (Measles), Mumps, Rubella: 2 vaccines or positive titer (exempt if born before 1957)
  • Hepatitis B and Varicella: Series, titer, or declination accepted
  • Flu Seasonal Vaccination or facility-specific declination form during flu season
  • COVID vaccination or facility-specific declination
• References: 2 professional references within 2 years of start
• Employment Application: Full 7 years of work history with no gaps
• Work History Verification: 7 years verified within 90 days of start

Additional Requirements

• Proof of current auto insurance required (due to driving responsibilities)
• Signed Orientation Safety Manual Acknowledgement (updated annually)
• Completion of facility-specific policies such as Acceptable Use Policy and Flu/COVID statements prior to start
