Required Documentation & Compliance

Identification & Verification

• Government-issued photo ID (Driver's License or State ID)

Background Checks

• Background check within 45 days of start, covering all locations lived/worked in past 7 years
• Must include: SSN Trace, Death Master Index, Counties, Nationwide, National Sex Offender, FACIS Level 3, Education Verification, MVR (if Home Health), and International Background Search if applicable
• Background checks updated annually; special rules for returning candidates with less than 30-day gap
• EPLS/GSA/SAM and HHS/OIG checks required within 30 days of start, updated monthly
• State-specific searches required for Washington and Oregon placements
• OBRA Registry Search required for CNAs in Washington at Skilled Nursing Facilities only

Certifications

• Required Certifications:

  • ACLS (AHA or Canadian Heart & Stroke Foundation)
  • BLS (AHA or Canadian Heart & Stroke Foundation)
  • PALS (AHA or Canadian Heart & Stroke Foundation)
  • NRP (Neonatal Resuscitation Program) if applicable
  • Chemotherapy certification if required by clinical track
  • Electronic Fetal Monitoring (EFM) certification for applicable specialties
  • IV Certification (LVN/LPN) as applicable
  • Fire Card (required; must be obtained prior to or within 30 days of start)
  • Management of Assaultive Behavior (MAB) if required
  • NIH Stroke Certification (renew every 2 years)
  • CPI (Crisis Prevention Institute) if required
  • State License and NURSYS verification required
• Certifications must be verified through the credentialing system or clinical tracks

Health Requirements

• Physical Exam within 1 year of start, signed by MD, NP, PA, or DO
• Fit Mask test (N95 or equivalent) required for travel starts after 2/5/2024, updated annually
• Immunizations and Titers:

  • Tetanus/Diphtheria/Tdap (within 10 years or declination accepted)
  • Negative TB test or Chest X-Ray within 12 months, updated annually
  • MMR (Measles, Mumps, Rubella) - 2 vaccines or positive titer or declination
  • Hepatitis B vaccination or signed declination (special rules for Alaska placements)
  • Varicella - 2 vaccines or positive titer or declination
  • Seasonal Flu vaccine or declination (required during flu season)
  • COVID vaccine or declination
• Drug Testing: 10-panel drug screen within 90 days of start (no instant tests)

Competencies & Skills

• Medication Test/Pharmacology Competency Exam as applicable
• Competency Exams required by specialty (e.g., Medication Aide exam for LTC CNAs in Oregon)
• Color Blind Testing if required by unit
• Skills Checklists updated annually for applicable units

Employment History & References

• Employment application covering at least last 5 years (7 years for APN/Subcontractors)
• Minimum of 2 supervisor references covering 1 year of employment in last 3 years (6 months for CNAs)
• References must be from supervisors and include month/year format for dates worked

Facility & System Specific Requirements

Training & Modules

• Completion of Providence Health and Services WorkRamp LMS modules prior to start:

  • Core Modules by profession and region (e.g., RN/LVN Core Modules for California)
  • Specialty-specific modules for ER, L&D, ICU, etc., as applicable
  • Annual regulatory modules required for all caregivers
  • Epic training modules required for SoCal and certain other regions (Per Diem only)
• Modules assigned and tracked through Aya's LMS platform, WorkRamp

Additional Facility Policies

• Float Policy Acknowledgement: May be required to float to other Providence facilities within reasonable distance
• Badge Photo: Passport-style photo with neutral background required for badge issuance
• Kronos Job-Aide & Attestation Form required for certain regions
