7-year background check completed within 30 days of start and updated annually
EPLS/GSA/SAM check within 30 days prior to start
HHS/OIG check within 30 days prior to start
New York State Medicaid Exclusion List check within 30 days prior to start and updated annually
Copy of diploma or transcript, or education verification
2 clinical references at supervisor level or above
Current skills checklist within the last year
Employment history covering 7 years with any gaps explained
Completed facility-specific AUP form
Facility-specific physical exam form completed by a physician within 30 days of start
Negative TB test or chest X-ray completed within 1 year of start
Measles, mumps, rubella, and varicella immunity or vaccination documentation
Hepatitis B antibody and antigen results
Standard 9-panel drug screen
Seasonal flu vaccination or documentation, with declinations allowed
COVID vaccine documentation if available

Physical exam must be initiated within 24–48 hours of offer acceptance
Drug screen must be initiated within 24–48 hours of offer acceptance
Fit testing is required within 12 months of start and cannot be completed onsite
Medication/pharmacology competency exam is required
