Other certifications may be position-dependent (e.g., CCRN, TNCC) – verify as needed

7-year background check including SSN trace, county searches, sex offender, education verification, and FACIS Level 3 screening
EPLS/GSA/SAM, HHS/OIG checks required and updated annually
Education verification required (Bachelor of Science in Nursing preferred)
Drug screening: Negative 9-panel drug screen within 6 months (excluding marijuana due to CA law)
Physical exam within 1 year of start
Fit mask test within 1 year, renewed annually (with accommodations for religious exemptions or facial hair)
TB screening: Negative test within 90 days of start, renewed annually
Immunizations:
- Tetanus/Diphtheria/Tdap within 10 years
- Measles, Mumps, Rubella (MMR) immunity or vaccination
- Hepatitis B series or declination
- Varicella immunity or vaccination
- Seasonal flu vaccination during flu season (Nov 1 - Apr 30) or signed declination
Respiratory safety clearance required annually
References: 2 professional references within 3 years (Aya requirement, not client)
Full 7 years of work history required on application with no gaps

Must review and sign facility-specific documents including job description, standards of business conduct, code of conduct, vendor training, and regulatory orientation prior to start
Compliance with facility dress code and policies
Competency exams as applicable (medication test, telemetry, dysrhythmia/EKG)
Some positions may require pass/fail testing (e.g., EKG exam for ICU RNs)
